Follow instructions and please attach any useful informations (pdf, doc and docx files). This will help to get back automatically our UHM Emergency Medical Report Form and a Pre Admission Agreement Form. As soon as all the fields are filled out by the Physician and the Insurance Company, documents must be sent by Fax to the CALL CENTER . This will help us to assess the severity of the patient and payment guarantee. Please note also that on special request, a strong medical support with extra fee can be offered with a short notice for retrieval of some critically sick patients.
